New Member? Here's How to Get Started
Make sure the doctor on your ID card is the one you want to see.
- If you would like to change your doctor, you can use the ‘Find a Provider’ search on CanopyHealth.com to find a new doctor who meets your needs.
- You can request the change by calling your health insurance member services phone number found on your ID card.
For more information about your medical group, visit their website below.
Once your health insurance is effective, contact your doctor’s office to schedule your first appointment with your primary care physician.
Here is what you need to prepare for your first visit:
- Come to your appointment with a prioritized list of items you would like to discuss.
- Bring your health insurance card.
- Be prepared to pay a co-payment if needed.
- Bring a list of your current medications to the appointment.
If you are taking your child to their first appointment, remember to bring their immunization records.
It is important for your new doctor to know as much about you as possible. Transferring your medical records is the best way to do that.
- Ask your previous doctor for a records release form.
- Sign and submit the records release form to allow your previous doctor’s office to share with your new doctor.
- Your records will be sent directly to your new doctor’s office.
NOTE: Some doctors and medical groups may charge a fee to send copies of your records. If you have any questions about these charges, please contact the doctor’s office.
Canopy Health’s Alliance Referral Program gives you the choice to find a specialist that is right for you. In most HMOs you must get your care within your medical group. Canopy Health gives you access to all our specialists throughout the Bay Area, even those outside of your medical group.
- Schedule an appointment with your primary care doctor.
- You and your doctor will discuss the best option within the Canopy Health network.
- Your doctor will provide you with a referral to the appropriate specialist.
NOTE: OB/GYN services do not require a referral.
